1. Parkinson’s disease. 
2. Bed-ridden. She cannot take care herself. 
3. Dysphagia and poor appetite.
4. Pressure sores on pelvis and scapula.
5. Patient is agitated and depressed.
 
After Home Care
1. Able to try oral puree feeding daily.
2. Regular turning and dressing to keep skin dry and clean. Finally, pressure sores are healed. 
3. Patient becomes more optimistic. She desires to communicate with others.
 
Active Global Holistic Caregiving Plan
Basic Activities of Daily Life:
1. Basic Care.
2. Monitoring conditions and vital signs. 
3. Medication Compliance.
4. Turning patient’s position regularly. 
5. Keep bedding hygiene. Keep skin dry to avoid pressure sore.
6. Keep regular bowel movement.
 
Safety Management:
1. Set bed handrails for fall prevention.
 
Rehab Exercise:
1. Massage patient’s four limbs to prevent the contraction.
2. Under caregiver’s monitoring to do physiotherapy exercise to improve balance and mobility of patient. Rehabilitation exercises include balance control, stretching and muscle strengthening.
 
Psychological Aspect: 
1. Enhance the communication between patient and her family.
2. Communicate appropriately to avoid misunderstanding. This can enhance the relationship between patient and the caregiver.
