Community Care Service Voucher for the Elderly
On March 1, 2020, we officially became one of the Recognised Service Provider of the "Community Care Service Voucher for the Elderly". Voucher holders can now use a wide varieties of home care services provided by Active Global including: medical escorts, personal care workers, health workers, registered nurses, enrolled nurses, physiotherapists, occupational therapists, physiotherapy assistants, occupational therapy assistants as well as providing on-site nursing training services.
What Is Community Care Service Voucher For The Elderly?
In September 2013, the Social Welfare Department launched the "Pilot Scheme on Community Care Service Voucher for the Elderly (Pilot Scheme)", which uses a "money-following-the-user" approach to allow eligible elders to use the community care service voucher to choose appropriate community care service according to their personal needs. We are an Recognised Service Provider (RSP) approved service unit recognized by the Social Welfare Department.
All our Nursing Care Staff have clinical experience (in hospitals, nursing homes, clinics and one-to-one nursing care, etc.) and licenses issued by relevant institutions, such as:
Assessment Form
Applicants or their family are required to bring the "Elderly Community Care Service Voucher Certificate" to Active Global in order to understand and confirm the detailed service arrangements (such as service date, time, location and health conditions of the elderly, etc.) and sign relevant documents.
After confirmation of service, our nurse will conduct a free assessment for the eldelry and set up a "Individualised Care Plan". Our nursing staff will provide related care or rehabilitation exercises according to the "Individualised Care Plan".
After confirmation by both parties, nursing staff can be arranged to provide services on the days required by the eldlery. We will also provide follow-up services, you can feedback on the work performance of the nursing staff to us.
